Just fit the two pieces together directly over your testicles and secure it with the inset hidden screw using the included allen wrench. Love this ball stretcher! It feels so awesome hanging on my boys! I got the 40mm tall by 28mm id because I did NOT want to worry about it falling off. I have several other donut stretchers in 33mm id and they will eventually end up sliding off one ball at a time,especially if I am sweaty. It is a bit of work to get it on without pinching your sack, but I always try to have my balls soft and hanging anyhow, so the skin is quite pliable. Just gotta be careful. My only complaint I have seen mentioned a number of times in other reviews, and that is the tiny size screw that is installed in these. I can just see stripping out either the socket head of the screw or the threads themselves, and being unable to remove the weight. I can just see some people going to the emergency room or a doctor to get it removed, which would be a bit embarrassing. I would just get my cordless drill and drill the head off the screw so I could get it off. So within the first 2 days, I drilled out all the holes and tapped the female side to 1/4"-20 thread and put in a much more substantial screw. made me feel a lot more confident. Otherwise I love it!
